Just as we prepare for your visit by accessing your chart and reviewing your information, we recommend you prepare to ensure your visit is as successful as possible.

Preparation Tips

If this is your first visit, we will ask you to complete a New Patient Form, which will include questions about personal and family medical history, contact information, insurance, and any current medications you may be taking. We will call to confirm your appointment the day before.


  • Bring in a list of all your current medications, or bring your medication bottles to the office with you.

  • Bring your current medical insurance information.

  • Write down any questions you may have regarding the reason for your visit, including any changes since we saw you last.

  • Be prepared to pay any co-payment amount which may be due at the time of your visit.

Our Services

We specialize in family and geriatric medicine. We are devoted to comprehensive health care for people of all ages, providing continuing and comprehensive health care for you and your family. The work we do is based on our knowledge of you in the context of your family and community. We promote a healthy lifestyle, and place emphasis on disease prevention and management.


Our services include, but are not limited to:

  • wellness exams

  • immunizations

  • acute care

  • minor surgical procedures (i.e., excision of skin lesions)

  • management of chronic health conditions


FAA Medical Certification

Dr. Michael Daniels performs Class 2 and Class 3 Airman Medical Certification Examinations. To access and complete your FAA 8500-8 form online, please log into the FAA website: https://medxpress.faa.gov.

Our Policies

We want you to understand clearly how we work so you can get the very best of care, consistently. Our policies are in place to help us provide excellent care to you each time you visit.

Proof of Insurance: Please present your insurance card(s) at each visit.

Appointments: Same-day appointments are available for acute illnesses and injuries. Routine checkups and physicals are normally scheduled within one to three weeks, depending on the specific needs of the patient.

We ask that you arrive 10-15 minutes prior to your scheduled appointment time so we can update your information without delaying your visit with your provider.


Cancellations: If you need to cancel an appointment, please do so as early as possible so that we can provide another patient an opportunity to be seen.

Payment: Copayments are collected at the time of your visit. We accept cash, checks, Visa and MasterCard.

Privacy: Please review the HIPAA information in your chart regularly and notify us if you wish to change any of your contact information. We cannot provide health information to anyone who is not listed on your HIPAA page.

FAQs

You’ve Got Questions - We’ve Got Answers

Can I view my medical information online?

Yes. Log in to MEDENT using the login information provided to you by our office. Be sure to create your own, secure password the first time you log in.

Does your office accept my insurance plan?

We participate with most insurance plans. If you have any questions regarding your insurance coverage, please call our billing department to verify that your visit will be covered by your insurance.

How long will I have to wait to get an appointment?

If you have an acute illness, please call our office and we will make every effort to give you an appointment on that day. Routine checkups can be scheduled within two to three weeks and comprehensive physical examinations are scheduled within three to four weeks in advance.

What services do you offer?

Our patients range in age from newborn to geriatric, so we offer age-appropriate examinations and immunizations. We see patients for acute illnesses and injuries and monitor chronic health conditions such as diabetes and high blood pressure as well. Dr. Daniels sees patients during hospitalizations at Carlisle Regional Medical Center and at several local nursing homes, as well as at our office. He also performs in-office minor surgical procedures such as skin lesion removals and suturing of lacerations.

What should I do if I need medical care when the office is closed?

MDVIP members should call Dr. Daniels at his private number.

Is what I discuss with you confidential?

Yes. Rest assured that all the information you provide to our staff and doctor is kept in the strictest confidence. We believe an atmosphere of trust is essential in providing you with the best healthcare possible. Please review our Privacy Statement.

Will I be charged if I cancel my appointment?

We confirm all appointments within 24 hours and we request the courtesy of a 48-hour cancellation of all appointments. However, we realize changes occur that may be beyond your control and may make an exception where we believe an exception is warranted.
